Our home visit rehabilitation service is accredited by Shizuoka prefecture. We have licensed therapists to provide physical, occupational, and speech therapy in your home. Our licensed therapist evaluates home environment and surrounding area in addition to the health and physical condition to create personalized therapy plans for the best outcome.
What is Home Visit Rehabilitation?
Home visit rehabilitation provide rehabilitation therapy at your own home.
Our licensed physical, occupational, and speech therapist assess home environment and surrounding areas to create personalized therapy plan. They address concerns and requests such as "I cannot perform certain tasks after coming home from the hospital," and "I want to continue the rehabilitation after discharge from the hospital."
Exercise Therapy
Exercise therapy includes gate training and muscle strengthening exercise in order for individuals to be able to walk and perform basic tasks safely.
Upper Extremity Exercise
Upper Extremity Exercises helps with contracture and deformation of joints caused by broken bones and paralysis. The exercises target shoulders, hands, and fingers to increase mobility.
ADL Training
Activities of Daily Living (ADL) include bathing, toileting and other everyday tasks. Therapists provide exercises to improve patient's skills to perform those tasks.
Swallowing Training
Swallowing training includes advices on food textures and dietary recommendations, oral care, and swallowing training to support individuals with drinking, eating and swallowing disorders.
Articulation Disorder
Speech difficulties can be caused by motor speech disorders and neuromuscular disorders. Pronunciation exercise, oral function and pronunciation practice are available for individuals with articulation disorders to improve their speech.
Aphasia
Aphasia is caused by the damage or the injury to language parts of the brain. Aphasia is a communication disorder, and affects speaking, understanding, reading, and writing. Speech therapists provide exercise and trainings, such as naming objects, reading and writing.
Swallowing Disorder
Speech therapists provide advices and exercises for swallowing, cough management and oral care for individuals with swallowing difficulties.
Other
Living with pain and paralysis caused by illness and injury is physically and emotionally painful. Our goal is to reduce the pain level to improve quality of life by providing joint mobility exercises and massages.
We have assistive device consultants to make recommendations for durable medical equipment and remodeling the house to support living in your own home.

Coordination with Care Manager
Care Manager will contact service providers to coordinate the care.


Doctor’s Order
Once the doctor’s order is received and the care coordination is completed, the care manager contacts patients with the care plan.


Signing a Contract
Patients sign the contract and the service begins.
