1. Application for certification of long-term care
The care manager performs an application for the certification for long-term care and renewal procedures.
2. Introduction to care insurance facility
If the user wishes to enter or enter the Long-term Care Insurance Facility, we will introduce referrals to the Long-term Care Insurance Facility and other facilities.
3. Long-term care certification
We will create a care plan, taking into consideration the wishes of the users and their families. We will consider the appropriate service together.
4. Start service
Coordinate with the service provider and start the service.
5. Review of care plan
Review the care plan as appropriate according to your physical condition and the care situation.
